First, and most importantly, as noted in the "What is This?" post, all characters should be motivated to explore dungeons, uncover ancient secrets, slay monsters, and gain treasure. Also, all characters should be willing to work with whoever joins the party. It's up to you as a player to come up with a concept that allows for this, but I as the DM am more than willing to work with you on coming up with a concept and fitting it into the campaign.
Books
We will be using all of the official books:
Basic Rules
Players Handbook
Elemental Evil Players Guide
Sword Coast Adventurers Guide (note that the campaign does NOT take place on the Sword Coast, so none of the "fluff" applies)
Volo's Guide to Monsters (only the Player Character Races section, and, again the campaign does not take place in the Forgotten Realms, so FR-specific "fluff" doesn't apply)
The Ranger, Revised Unearthed Arcana (you can use either this revised version of the ranger or the ranger as presented in the PHB)
Ability Scores
This isn't Old School, but sometimes the Old Ways aren't the Best Ways. Every character will use the Default Starting Array: 15, 14, 13, 12, 10, 8, arrange as desired. Remember your racial bonuses!
Backgrounds
Any Background from any of the official books is ok. My preference is for everyone to have a different background, for differentiation and niche protection, but if all players are ok with it, there's no rule against more than character having the same background.
We won't be using customized Backgrounds for this campaign, but you can change the "fluff" of an existing background to taste.
Level
You start at level 1.
Really?
Yes. Old School. Zero to Hero. Level 1. But, as I am old and soft, if you are replacing a deceased or retired character, your new character starts at the same level.
Starting Gear
You start with the basic starting gear from your class and background. We will not be using the "a la carte" option.
Feats
We will be using Feats, with the following campaign rule. No more than one character can have the same Feat, with the following exceptions.
Elemental Adept: each character must choose a different element.
Resilient: each character must choose a different Ability and saving throw.
Weapon Master: each character must choose a different set of weapons.
Note that Variant Humans (in the Players Handbook) start with a Feat; all other characters have to wait for their first Ability Score Increase to get a Feat.
